The Outcome

Through the two phases of funding to date we have sought applications from organisations to develop and implement programmes and activities that will contribute to the delivery of the long term outcome for the intervention project which has been defined as:

'People (and their households) in County Durham aged 40-74 with an estimated and actual risk calculation of cardiovascular disease (CVD) greater than 20% increase their level of participation in physical activity.'

We have incorporate households into the definition, which means that the families of the CVD target group are also included to broaden the potential scope for organisations wishing to apply for funding.
