The Outcome
Through the two phases of funding to date we have sought applications from organisations to develop and implement programmes and activities that will contribute to the delivery of the long term outcome for the intervention project which has been defined as:
'People (and their households) in County Durham aged 40-74 with an estimated and actual risk calculation of cardiovascular disease (CVD) greater than 20% increase their level of participation in physical activity.'
We have incorporate households into the definition, which means that the families of the CVD target group are also included to broaden the potential scope for organisations wishing to apply for funding. This provides and opportunity to develop approaches that include family based activities as part or all of the project application.

Encouraging people to participate is important of course, but we want to go further than that and we will therefore be measuring the success of the project by the number of the target group who sustain an incraese in participation 6 months after starting the intervention.
In Phase 2, we placed a further emphasis on submissions from organisations to achieve the following:
- Additional service provision for the CVD risk target group in the ex-districts of Sedgefield, Teesdale and Wear Valley (with strong links to existing exercise referral pathways)
- Interventions that focus on encouraging those within the CVD risk target group and also their family members to participate together
- Projects that deliver one-to-one or group support and also couple this support with other modes of support, for instance: telephone, online support and other feedback systems/media (example only)
- Projects that also encourage or provide popular modes of activity such as walking, running, cycling and swimming and that take a coordinated approach to planning i.e. identifying existing and planned provision in the rationale for the project
